At 7 am, a tanker-lorry delivered ammonia to a chemical plant. The driver and 1 technician connected the cistern transfer hose (DN 80 mm) to the pump suction hose (DN 60 mm) in using a flange. Next, the valve was opened on the pump side in order to test the assembly’s seal. Since the suction hose had been pressurised subsequent to a pump malfunction and since the adapter had been poorly clamped, 20 litres of a 20% ammonia solution escaped, causing breathing difficulties and eye irritation to the 2 workers at the scene. Only the driver, who was struck by flying debris in the eye (despite wearing glasses), was ordered off work for 4 days.
