One of the wagons of an empty convoy (6 wagons) used for transporting benzene derailed while building up a set of wagons in the grounds of a refinery. The manoeuvre to be carried out involved pushing a convoy of 17 empty wagons, in order to make room for 9 others, 2 of which were in the approaching set of wagons. For operational reasons, the team deviated from the usual manoeuvring procedure. This manoeuvre, usually controlled by an external technician, was controlled on that day by the coordinator from his office by radio. However, the coordinator was distracted by another operator during the operation and his attention wavered. The last wagon on the track ran into the stop, tipped it over and rode over it. The coordinator noticed this and activated an emergency stop; the wagon found itself balancing on the bumper and an axle came loose.

LPG transfer pipelines were situated nearby. An external company lifted the wagon in order to put it back on an isolation track a few hours later. The incident had several causes: confusion in the roles of the people involved in the manoeuvre, and insufficient or inconsistent communication between them during the manoeuvre, etc. Furthermore, the operator is carrying out a study aiming to place the bumpers further away from LPG lines.
