At a quarry, a temp worker ascended a conveyor belt to lubricate another conveyor. For an unknown reason, he fell from a height of 1.50 m onto a concrete slab and fractured his right elbow. He was placed on a 3-month leave. The unsecured access to lubrication points, plus the lack of any individual or group protection when working above ground, insufficient information communicated on the risks of working at height and the absence of a specific operating protocol were all cited by the quarry operator as factors contributing to this accident. A memorandum on the accident, in the form of a summary document, was disseminated to all Group sites, and the lubrication point was moved in order to prevent hazardous work situations from recurring. After identifying other on-site zones potentially at risk for working at height, various additional safety features were implemented.
