Inside a tyre factory, an employee was working on a hot water pipe cleaning out an extrusion head. Subsequent to a shock with a pneumatic screw gun, the pipe broke and the worker was burned to the 2nd degree on the abdomen, elbow and thigh by hot water. He was immediately hospitalised. The accident was caused by the weakened pipe coupling and technician oversight in failing to drain the pipes. Regular monitoring of the newly replaced coupling was performed while waiting to automate the pipe drainage step by introducing a tarpaulin prior to any intervention. The installation of solenoid valves and access gates was also planned. This was the second accident at the plant in as many days (ARIA base no. 30682).
