One man was killed and 2 others injured (broken leg) when a 12 m tank exploded during maintenance work at a refinery. The 52-year old contactor was installing a device on the top of a 3-m-wide, cylindrical tank with a torch gun when it exploded, peeling the roof back. The tank is believed to have contained lime slurry (polluted with hydrocarbons?), which is used as part of the water treatment process in the plant’s cogeneration unit.
