In a warehouse storing chemical products, a big-bag fell during the loading of a hopper following the failure of a hoist unit on the fixed rope side. The hopper was positioned on a 5 m-tall platform. Using a beam-mounted hoist, a technician was moving the big-bag over a distance of 10 m at a height of 50 cm above the platform floor, as the height of the building prevented it from being raised any higher. He then positioned it above the hopper and started to open the emptying chute when he heard a crack. He removed his arm just before the bag fell. Production was stopped for one week – the time required to change the equipment. According to the operator, the fixed rope could have been stressed by the torque limiter used as the upper limit switch. The fixed rope pin had previously failed three months ago. The operator is equipping the unit with a hoist with a limit switch, using smaller big-bags, implementing preventive maintenance and planning to raise the building so that the big-bags can be placed on a structure during emptying and thus improve the ergonomics of the work station.
