This fact sheet is the analysis of some accidents that illustrate the risks associated with the operation of installations in degraded mode (unavailability of equipment, utility, personnel …). These degraded operating situations can arise from material failures, but also from organizational and human failures. In order to avoid accidents, and in the absence of being able to avoid degraded operation by appropriate arrangements, a specific road plan must be defined to carry on operations under a safety level equivalent to the reference.
