A surface drill rig was being used to make an initial cut into a narrow rocky spur in a solid-rock quarry. The area around the surface drill rig offered no flat strip of ground in order to move around the rig. After drilling a second hole, the operator got out of the rig. He was wearing a hard hat and holding a 10-metre tape in order to measure the depth of the hole. His foot slipped on the platform and he fell 15 m off the working face. He suffered a bruised head, a bruised spleen (non-serious), a fractured left wrist, and a fractured pelvis. He was not wearing a safety harness.
