At 7 am in a Seveso-rated cosmetics plant, the operator on the lacquer case filling line noted the pooling of a liquefied butane/propane mix on the floor. She shut down the line and staff evacuated the premises. The gas leak stopped and the building was ventilated. The leak was due to rupture of a hose in which the gas mix had circulated under 9 bar of pressure. Maintenance staff replaced the hose. The alarm was lifted around 7:30 am.

The damaged hose had been changed 3 days prior. It turns out that this hose was not adapted for use with liquefied gases. The plant operator identified the hoses with compatible materials and stored them in a designated warehouse zone. Specifications were also determined for future equipment orders. During the evacuation sequence, a number of employees spontaneously stood at the front gate to block access to the plant; this action was incorporated into the evacuation protocol.
