In the workshop of an epoxy resin and paint plant, fire broke out following the temperature rise of a mixer containing 500 kg of resin. The blaze caused a major smoke release. The workshop was evacuated, and the site’s safety crew quickly brought the incident under control. The 600-litre mixer was immersed in order to stop the runaway reaction and placed in a retention facility. One slight injury was treated by the company’s infirmary. Several neighbours experienced mild irritations to the eyes and throat. Subsequent to this accident, the mixer was shut down for several days, i.e. the time necessary to clean and repair both the device and its premises, revalidate existing guidelines, and restore the associated utilities and networks. The accident was due to a poor transmission of heating instructions during the midday shift change. A detailed action plan was implemented, calling for: repositioning the rupture discs, studying the thermal stability of the mix, optimising the process, and using a safety valve that required technicians to recheck heating at predetermined intervals.
